BIG DEAL 2025-26 Booking & Consent Form

To book a place for Big Deal 2025-26, this form must be completed and signed by the participant's PARENT or GUARDIAN, on behalf of the young person. If you are booking for more than one child you must complete a separate form for each child.

I, {nameOf} (Parent/Guardian), give permission for my child, {nameOf7}, to attend Big Deal projects and events at The Galleon Arts Centre. I understand that they will be in the care of Big Deal and The Galleon Arts Centre, and while The Galleon Arts Centre will take all reasonable care of the children, they cannot neccessarily be held responsible for any loss, damage or injury arising during or as a result of the project.
I understand that if my child leaves the site/workshop premises during the session, or makes their own way to or from the site/workshop venues, they do so at their own risk, and during this time is not the responsibility of Big Deal and The Galleon Arts Centre.
As part of the Big Deal project there will be opportunities to attend off site trips and events. I give my permission for my child to attend these trips/events. I understand that Big Deal project managers will notify me of the details as and when they arise, and I will be required to return a consent form for each off site event.
I give permission for my child to be given basic first aid in the event of an accident while they are participating in Big Deal activities, and for administering prescribed medicine provided only by myself. I agree to make Big Deal project managers & leaders aware of any medical conditions or access needs that might affect my child’s health and wellbeing. In the event of an emergency, I consent to a Big Deal or Galleon Arts Centre authorising medical treatment at hospital, should I not be immediately available, and should waiting to obtain my permission be medically inadvisable.
I have read and understood the above statement and agree to my child participating in Big Deal under these terms. I understand that without my signed consent at the end of this document, my child cannot take part in Big Deal.*
