Turkey Hoops Basketball Tournament - Participant Waiver and Release of Liability* This field is required.I, the undersigned participant, hereby acknowledge and agree to the following terms and conditions as a prerequisite for my participation in the Turkey Hoops Basketball Tournament organized by Turkey Hoops, a non-profit organization (hereinafter referred to as "the Event"). 1. Assumption of Risk I understand and acknowledge that participation in the Event involves inherent risks, including but not limited to physical injury, accidents, illness, and property damage, some of which may result from the actions, inactions, or negligence of myself or others, including participants, volunteers, or spectators. I knowingly and voluntarily assume all such risks, whether foreseen or unforeseen, including those related to the playing of basketball or any other activities associated with the Event. 2. Physical Fitness Certification I certify that I am in good health and physically capable of participating in the basketball tournament. I have no known medical conditions that would affect my ability to participate safely. If I have any pre-existing conditions, I have consulted with a healthcare provider and obtained medical clearance to participate. I acknowledge that it is my responsibility to monitor my own condition and well-being during the Event. 3. Release of Liability In consideration for being allowed to participate in the Event, I hereby release, waive, discharge, and covenant not to sue Turkey Hoops, its directors, officers, employees, volunteers, agents, sponsors, representatives, and any other affiliated entities (collectively referred to as "Releasees") from any and all liability, claims, demands, actions, or causes of action of any kind or nature, whether known or unknown, arising out of or in connection with my participation in the Event. This release applies to any loss, injury, damage, or death that may be sustained by me, including injuries arising from the negligence of the Releasees, to the fullest extent permitted by law. 4. Indemnification I agree to indemnify and hold harmless the Releasees from any and all claims, losses, liabilities, damages, or expenses, including reasonable attorney's fees, arising from or in connection with my participation in the Event, including any third-party claims arising out of my actions or omissions. 5. Photography and Media Release I grant permission to Turkey Hoops and its designated agents to capture and use photographs, video recordings, or other media of me during the Event for any lawful promotional, marketing, or informational purposes, in any and all media, without further consent or compensation to me. I waive any rights to inspect or approve the use of such materials and any associated content. 6. Agreement to Follow Event Rules I agree to comply with all rules, policies, and guidelines provided by the Event organizers and will follow all instructions given by Event staff, volunteers, and officials. I understand that failure to adhere to these rules or engage in unsafe behavior may result in my immediate removal from the Event without refund or recourse. 7. No Alcohol or Drug Use I affirm that I will not consume alcohol or use any illegal substances before or during the Event. I understand that any violation of this policy may result in my immediate removal from the Event. 8. Opt-In for Electronic Communication I consent to receive electronic communications from Turkey Hoops, including information about future events, updates, and promotional materials. I also consent to receive communication from sponsors of the Event via phone, text message, or email. I understand that I can opt-out of receiving such communications at any time. 9. Severability If any provision of this waiver is found to be invalid, illegal, or unenforceable, the remaining provisions will continue in full force and effect. Any invalid or unenforceable provision will be deemed modified to the extent necessary to make it enforceable, without affecting the remaining provisions of this waiver. 10. Acknowledgment of Understanding I have carefully read and fully understand the terms of this Waiver and Release of Liability. I acknowledge that by signing this document, I am voluntarily waiving certain legal rights, including the right to bring a lawsuit against the Releasees. I sign this waiver freely and voluntarily, intending to be legally bound by its terms.
