This form MUST BE completed and approved prior to all trips of 3 nights or more. See Safety Activity Checkpoints and Troop Activity Guide for trip and activity planning information, including filing dates, girl/adult ratios, etc.
Before you head out on your fun-filled activity, remember to complete and submit the forms by the required deadlines;
-Overnight trips within the US- 3 months prior
- International Trips- 2 years prior
Participants: Estimate the number of participants and list by level.
Adult leadership:
(Two approved, registered and trained leaders must attend all Girl Scout gatherings, meetings, and field trips)
Transportation: Types of transportation for this trip/activity. Check all that apply below.
If renting/leasing a vehicle, GSME's "Council Rental Agreement" must be completed and returned to GSME 3 weeks prior to the trip.
Insurance: Additional insurance is required for any trip that is planned for 3 nights or more. Please contact customercare@gsmaine.org to purchase insurance.
Girls should have experience at every level of travel progression before moving on to the next level. For regional travel, girls must be Juniors and older. For national trips and trips to Canda, girls must be Cadette level or older. For international trips, girls must be Seniors and older at the time of travel.
