General Terms and Conditions:
(1) Once Administration has approved your application, you will be expected to provide a copy of your Social Security Card. Residents of Buttonwood Bay will be provided with a plastic membership card which must be presented while paying for services.
(2) Only one application is required per household. The membership card will be assigned to the individual that submitted the application. If the patient’s name does not match the last name of the registered member, we will request further approval to apply benefits.
(3) The neighborhood program grants a 12% discount on Laboratory and Diagnostic Services to residents of Buttonwood Bay. Valid ID and membership card will be requested.
(4) In addition, being a member of the neighborhood program allows you to earn redeemable points at the BBMC pharmacy. These points can be applied during your next visit. When you register for the neighborhood program, Administration will automatically add your information to the pharmacy’s POS. When making a purchase, present a valid ID to cashiers. Points will be awarded after your sale.
(5) At this time, the membership card cannot be utilized for hospital services/procedures and pharmacy purchases. There are no discounts available on consultations.
(6) Patients cannot apply benefits of the membership card along with other promotions available to the public. The highest discount available will be applied to the sale, if applicable.
(7) BBMC reserves the right to amend these general terms and conditions.
(8) If you lose your membership card, a $15 fee will be charged for replacement.
