RELEASE OF LIABILITY, MEDICAL AUTHORIZATION, AND PARENT AGREEMENT
By signing below, I, the undersigned, hereby release and hold harmless Hosanna Performing Arts Foundation, including its directors, officers, agents, employees, volunteers, and representatives (collectively, “the Foundation”), from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, illness, or damage sustained by my child or myself while participating in rehearsals, activities, or performances associated with the 2026 production of Wintertide, whether occurring on the Foundation's premises or off-site.
In the event that medical treatment becomes necessary for my child (or myself), I understand that reasonable efforts will be made to contact the emergency contact(s) listed on my child’s registration form. However, if those individuals cannot be reached in a timely manner, I hereby authorize the Foundation and its authorized representatives to secure and consent to such medical treatment as may be deemed necessary under the circumstances, including but not limited to treatment by licensed medical personnel, hospitals, or emergency services.
I acknowledge and agree that I am financially responsible for all costs incurred as a result of any such medical care or treatment, and I further agree to indemnify and hold the Foundation harmless from any liability related to said medical expenses.
I further acknowledge that, despite reasonable precautions taken by the Foundation and its representatives, my child may be exposed to allergens or environmental irritants during rehearsals or performances, including but not limited to food products, stage makeup, and hair products. I waive any and all claims, demands, or actions against the Foundation for any reaction or injury resulting from such exposure. I understand it is my responsibility to provide suitable alternatives for my child if he or she has specific sensitivities or allergies.
PARENT AGREEMENT
I understand that parent or guardian involvement is critical to the success of the production. Accordingly, I agree that I, or a representative for my child, will attend all required parent meetings, and participate in volunteer activities as outlined by the Foundation.
MEDIA RELEASE
I furthermore give Hosanna Performing Arts Foundation, and its vendors authority to use my child’s image in photographs, videos, and web pages for promotional purposes.
