Due to precautionary measures for COVID-19, HRA is not conducting in-person appointments until further notice. If you are in need of assistance from one of our Case Managers, please answer the following questions to the best of your ability. NOTE: You will need to fill out this survey on a smartphone or computer. You will also need access to WiFi, as well as a working phone number and email address.
