Welcome! I can’t wait to meet you! All guests start here with a digital consultation form. This allows us to get to know you and your hair goals so that we can ensure your appointment is booked properly. Please complete the form in full and I’ll be in touch within 24 hours of your request during my work days which is Tuesday-Saturday. By signing this form, I confirm that I have read and understand the following information.

*I confirm that I have not used any Henna, Sun In, or any other product with metallic salts in my hair. (PLEASE NOTE - Metallic salts in some of these products CAN cause very unpredictable results, reactions, melting hair, unexpected colors, etc, PLEASE let me know if you have ever used any products relating to these, or if you have questions about it at all before we do a chemical service on your hair)

*I acknowledge and hold myself accountable for any negative outcomes of my hair if I have lied about any previous chemical services, withheld information over my hair, or withheld medications/treatments/conditions that may affect my hair during any chemical services.

*I understand that my hair is different than the hair I may be showing in inspiration photos. I understand that my inspiration photos will give my stylist a guide for the cut or color I would like my hair, but that in no way may my hair be a replica of this photo. I also understand some hair goals are a process, and may take multiple sessions to achieve desired outcome.

*I confirm that the salon/stylist will not be responsible or liable of chemical reaction, allergic reaction, and if the result of the service is not as expected. Everyone has different hair, and outcomes can be unpredictable sometimes when working with hair products and chemicals.

*I understand that my stylist cares about both me and my hair, and takes my hair integrity first and foremost into account while doing this chemical service. I understand that if at any point my stylist believes the integrity of my hair will be compromised, the service will immediately be stopped regardless of our end goal.

*I understand that proper draping techniques are always used during chemical services, however things do happen, and I do not hold my stylist responsible if clothes or personal belongings are damaged during services. I understand that it is recommend to change into an old T-shirt and clothes before coming to your appointment.

*I understand that if I do not follow a haircare routine provided by my stylist, the results and longevity of my service can be affected and I do not hold her accountable for this.

*I consent the salon to take photographs/video of the provided service, as well as sharing the photographs to social media for marketing or testimonials.
*I understand that I may also be asked to come in for an in person 30 minute consultation ($40) if needed to provide more accurate information for my appointment.

*CANCELLATION POLICY*

*I understand that we have a strict 48 hour cancellation policy. If I need to cancel, reschedule, or change service to a different service under the 48 hours, I will be charged the cancellation fee of 50% of the service scheduled to the card on file.

*I understand that if I show up later than 15 minutes to my scheduled appointment, there will not be enough time for the appointment, and the cancellation fee will be charged.
