All referrals must be for pregnant mothers or mothers of infants less than 5 months of age. If you have any questions, please call 901-287-4708. Fax: 901.287.4738
The HFT program is free of charge and services are offered for up to 3 years. Services can be extended to 5 years for caregivers who want the continued support. The program is strictly voluntary. To participate, the applicant must be a U.S. citizen or legal resident and provide proof of citizenship or legal resident status. Click Here for examples
