Photo Guidelines

Your photo ID badge awaits, but before completing this form, please review these guidelines to ensure your photo meets the criteria. Any photos submitted that do not meet the requirements will not be accepted.

Guidelines:

No selfies
No group photos
No photo collages
Clear, high-quality photo of the individual
Smile! You'll have your ID Badge in no time
Physical photos and scanned IDs will not be accepted
Hands and fingers should not cover the person's face.
No face coverings such as sunglasses, masks, costumes, etc.
The photo must be current. (Photo taken within the last 30-60 days)
Only the individual who needs the photo ID badge should be photographed.

All Youth Photo ID Badge form completed by Wednesday at 5:00 PM will be available for pick-up on Friday. A parent or guardian must be present to pick up the card, children will not be allowed to pick up their own cards.

Any residents between the ages of 14-17 accessing Heartland amenities without an adult must have BOTH the mobile access credentials and the Heartland Youth Photo ID in order to access amenities and may be asked to show both by Heartland Staff.
Any requests for a youth photo ID badge must be a current occupant in the home.
A digital waiver must be completed and signed by a guardian for any 14-17 y/o requesting a youth photo ID badge.
Youth photo ID badges will not be issued for residents with a balance on their association account.

Any 14-17 y/o requesting a youth photo ID badge must be listed on the current lease.
Each youth photo ID badge will be $10/per badge.

Upload a current photo for all listed 14-17 y/o residents requesting a youth photo ID badge. The file name must match the youth's full name and be saved as FirstNameLastName (**DO NOT UPLOAD A PHOTO OF ANOTHER ID, IT WILL NOT BE ACCEPTED AND WILL CAUSE DELAYS IN RECEIVING YOUR YOUTH ID.**). *

To avoid processing delays, be sure your photo meets all the following requirements. The photo must have been taken less than six months ago. The head must face the camera directly with a full face in view. No sunglasses may be worn in the photo. Use a plain white or off-white background. Pictures or scans of ID cards will not be accepted.
