PHOTO AND VIDEO PERMISSION AGREEMENT
By completing this enrolment form, I acknowledge and agree to the following regarding the use of photographs and videos of myself/my child(ren) taken during BCMA classes, performances, productions, and/or events.
Consent and Usage
Understanding that the Ballarat Centre of Music and the Arts (BCMA) prioritises child safety and adheres to high standards in this regard, I grant permission for BCMA to use photographs and/or videos of myself/my child(ren) for purposes including, but not limited to:
Promotional Materials:
Use in media releases, brochures, and on online platforms such as the BCMA website and social media channels.
Performance Recordings:
Inclusion in performance recordings for sale or sharing via platforms such as the BCMA YouTube channel, and other materials used for marketing or fundraising purposes.
Editorial Content:
Accompanying media editorials and occasional third-party advertising directly associated with BCMA.
Important Notes
I understand that if I do not provide consent for photo and video usage, myself/my child(ren) participation in certain performances and/or events may be limited.
In such cases, I acknowledge that specific photo and video permissions will be required for each BCMA activity. I may be asked to complete and sign separate consent forms for my child(ren) to participate in specific BCMA performances, productions, events, or promotional activities.
Should I choose to decline general consent for photo and video use, I will notify the BCMA office in writing via email.
This agreement reflects BCMA's commitment to maintaining transparency and safeguarding its participants while celebrating and promoting their achievements.
TERMS AND CONDITIONS SUMMARY AND EXTRACT

Medical Assistance
I consent to BCMA seeking medical assistance or calling an ambulance in an emergency and agree to cover any associated costs.
Enrolment Duration
Enrolment continues from the start date until the end of the calendar year unless cancelled as outlined below.
Cancellation Policy
A minimum of five weeks’ written notice must be provided to the BCMA Office to cancel an enrolment after it has commenced.
Term Fees
Fees must be paid in full prior to the commencement of each term, unless an authorised payment plan has been established.
Students participating in a trial group lesson are not required to pay fees until after their enrolment has been confirmed. Enrolment confirmation will occur in the week following the trial. If you do not wish to continue after your free group trial, please notify the office in writing. If we do not receive any communication from you, we will assume you wish to proceed with your enrolment, at which point all enrolment conditions will apply.
Outstanding Fees
Students with unpaid fees will not be permitted to attend classes until all debts are cleared. Any additional fees incurred in the process of debt collection will be added to the account.
Make-Up Lessons Policy
Group Classes: Make-up lessons or credits are not available for missed group classes (exceptions outlined in the full Terms & Conditions).
Private Lessons:
- Absences due to illness: A minimum of 4 hours' notice is required to qualify for a make-up lesson.
- Planned absences (e.g. school camps): A minimum of 4 working days’ notice is required to qualify for a make-up lesson.
- In the event that two or more lessons are missed due to illness within a term, a Doctors Certificate will be required in order to arrange any compensatory lessons or refunds. Otherwise, lesson fees will be due in full for any lessons not attended.
- In the event that offered make up lessons are refused after three attempts, any missed lessons will be an absent paid.
If the BCMA is unable provide a teacher for a private lesson, a substitute lesson or credit will be arranged.
Holding Fee for Long-Term Absence
Group Classes: A holding fee of $5 per group class per week applies for absences exceeding three weeks applies. Requests must be submitted in writing to the Director for approval a minimum of three weeks in advance.
Private Lessons: Lesson timeslots cannot be held for absences longer than three weeks.
Planned absences of longer than one week must be communicated in writing before term invoices are issued.
BCMA TMA - Talent Management Agency
I acknowledge that BCMA’s Talent Management Agency (TMA) may occassionally submit students for auditions and industry media and acting jobs, and I agree to Policy Clause 14 regarding this service if we choose to participate.
Child Safety Standards
The BCMA complies with Victorian Child Safety Standards as an approved Dance Safety Tuition Centre. Comprehensive Child Safety Policy & Procedures document and other relevant documents are available on request. A Current Child Employment Licence is held and all regulations strictly adhered to.
All adult volunteers, staff, contractors etc. are required to complete all relelvent documents and to hold a valid Working With Children Check, VIT and/or Police Check.
Assumption of Risk and Communicable Diseases Waiver
Participation in BCMA activities may involve potential exposure to communicable diseases (e.g., COVID-19).
While BCMA adheres to safety protocols, it cannot guarantee protection from exposure or infection.
I assume all risks associated with participation and release BCMA from any claims related to exposure or infection.
In the event of government restrictions, BCMA classes may transition to online. Fees will provide access to online classes, and refunds will not be provided unless written withdrawal notice is submitted as per the five-week notice period.
