Once submitted, Dr. Caccopola will review your information and email her commentary within 5 business days. It will outline how Functional Medicine and hormone optimization can be used to address the potential root causes of your symptoms and help you regain your quality of life.
As space becomes available in Dr. Caccopola's patient panel, we will contact waitlisted individuals in the order their questionnaires were received. At that time, we will invite you to schedule a complimentary 15-minute introductory phone call. This session allows you to discuss your symptoms and determine if our Wellness Programs are the right fit for your medical needs before enrolling.
