Barber & Beauticians Registration Form

I, Full Name* , agree to participate voluntarily at the Back to School Bash organized by HeritageU: Youth Empowerment Coalition, scheduled to take place on August 3, 2024 at Living with Purpose. I understand and acknowledge the following terms:

1. Donation of Services: I am donating my professional services as a barber/beautician for the duration of the event without expectation of monetary compensation. I understand that sales from products and voluntary tips are permissible.
2. Charitable Contribution: My participation and donation of services are intended to benefit the attendees of the event and support the objectives of HeritageU: Youth Empowerment Coalition.
3. Duration and Scope: I agree to provide services only during the designated hours and within the scope outlined by the organizers of the event.
4. Release of Liability: I release HeritageU: Youth Empowerment Coalition, its officers, directors, employees, and volunteers from any liability for injuries or damages that may occur during my participation in the event.

I acknowledge that my participation is voluntary and that I am under no obligation to provide services beyond what is agreed upon in this agreement.
