Please submit this form to refer a Georgia K-12 student to our free haircare program. If you are not an authorized referrer, you may not request free haircare on a student's behalf. Instead, the parent/guardian of the student may book an affordable hair appointment at www.higherfoundation.org/salon.

 

Eligibility Criteria:

• Must be currently enrolled in a Georgia public school in grades Kindergarten – 12th grade.
• Must have a demonstrated need of free hair care services that can be verified by a Georgia educator, social worker, or religious leader.

 

Application Requirements:

• (Authorized Referrer) Proof of employment to confirm he or she meets one of our referral requirements as listed below:
  • Georgia Principal
  • Georgia Assistant Principal
  • Georgia Teacher
  • Georgia Paraprofessional
  • Georgia School Counselor
  • Georgia Social Worker
  • Georgia Pastor or Youth Pastor
• (Student) Proof of enrollment in a Georgia K-12 public school with one of the following documents:
  • An unofficial school transcript for the current academic year
  • A progress report or report card for the current academic year

Application Period:

August 15th through April 15th for the open academic year.
