Higher Heights is open to youth ages 11–14. As space is limited, registration will be on a first-come, first-serve basis. All participants are strongly encouraged to bring their own lunch and drinks.

I hereby give permission to my child to attend the consecutive six (6) week (July 12, 19, 26, August 2, 9, and 16) basketball camp sponsored by Higher Heights, a ministry of Oversea Chinese Mission. With full knowledge of the competitiveness of the sport or sports in general, I will not hold Higher Heights nor Oversea Chinese Mission liable for injuries incurred during active participation of the camp. I also will not hold Higher Heights nor Oversea Chinese Mission liable for incidents incurred before or after the basketball camp during the said days when the camp is in session. Should there be injuries involved throughout the duration of the camp, I give authority to the staff of Higher Heights to obtain emergency medical treatment for my child with the understanding that I and/or another designated family member be notified as soon as possible.

I will assume the responsibility of either bringing my child to and from the school gym at Manny Cantor Center (197 E Broadway, New York, NY 10002) or allowing him/her to arrive and leave by himself/herself. In addition, I give unrestricted permission for the members of Higher Heights and Oversea Chinese Mission (154 Hester Street, New York, NY 10002) to bring my child from the school gym at Manny Cantor Center (197 E Broadway, New York, NY 10002) to Oversea Chinese Mission for any particular occasion during the six (6) week duration of the camp.

I am also aware that food and/or drink may not be provided during the days when the camp is in session. Although a t-shirt is provided free of charge, I am aware that proper active attire such as shorts and basketball sneakers are requested to ensure the overall wellbeing and safety of my child.

I also give permission for Higher Heights to use the photographs/videos taken during the program for future promotion of the camp.
