2022 Men's Masters Invitational EOI - For Individuals

NB. This form is for registration of INDIVIDUALS that wish to join an existing team for the 2022 Men's Masters Invitational Events to occur 25th - 27th February (34+ to 55+) and 18th to 20th March (60+ and above) at Sydney Olympic Park. Teams that are looking to express their interest to join a team should use the EOI - For Teams instead: https://form.jotform.com/HockeyAustralia/2022-mens-masters-eoi-teams. Please note at this time as per NSW Public Health Orders all entrants to a Sydney Olympic Park venue will be required to show proof of double vaccination in order to gain entry. EOI's will close on January 10th, 2021.
