I acknowledge that participation in WSB involves physical activity and I hereby release Hoopdreamz Enterprises Pty Ltd, and its staff from any liability for injuries or accidents that may occur during the events.

I am responsible for health and accident insurance and any medical costs incurred due to injury. You give permission for emergency medical transportation and treatment of your dancer(s) at your expense should the need arise.
