  • INTAKE INFORMATION 

    • Please complete and submit these forms ONLY if you are prepared to pay for and schedule your assessment within 48 hours.

    • Submitting the intake package will NOT impact your driving privileges. You MUST also pay for, schedule, and complete your assessment in order to maintain or restore compliant status (license eligibility) with the Department of Transportation.

    • If you are not able to complete the full payment and scheduling process at this time, please come back and complete this intake package when you are able to make the full payment.

  • Information about the Intoxicated Driver Program (IDP)

    The Department of Transportation (DOT) requires that a Driver Safety Plan (DSP) be developed at the Hope Council during your assessment. Based on the findings of your assessment, you will be required to attend either education or treatment, so you must be prepared for either. 

    *THE ASSESSMENT IS JUST THE 1ST STEP*

     

     Education Referral

    • If you are referred to education, you will be required to attend classes at Gateway Technical College (GTC).
    • You must enroll and pay in full for the required classes within 10 business days of your assessment.
    • Health insurance does not cover class fees, and payment plans are not available. Traffic safety school fees may not be reduced or waived per Wis. Stat. DHS 62.11.
    • The current rates are $303.70 for Group Dynamics and $435.40 for Multiple Offenders and are subject to change.
    • If you do not register within 10 business days, GTC will place your DSP into non-compliance with the DOT, and you may lose your driving privileges.
    • An outstanding balance with GTC will prevent you from enrolling in these or any technical college classes.   

    Treatment Referral                                                                  

    • A list of approved area treatment providers is included in this intake package; however, you may choose any Wis. Stat. DHS 75 licensed provider with prior agency approval.
    • If you are referred to treatment, you must inform the assessor of the provider you have selected during your assessment appointment. Failure to provide this information at that time may result in additional fees.
    • If you have health insurance and intend to use it to offset the cost of treatment services, it is your responsibility to contact your carrier to confirm which approved providers are covered before your assessment.
    • If you do not have health insurance that allows for substance use treatment services, you may be eligible for County Funding for Treatment Services if you meet program qualifications.

    County Funding 

    To determine whether you are eligible for County Funding for Treatment Services, you must bring all the following documents to your assessment appointment. If these documents are not provided, you may not be eligible for funding. These documents are only required if you do not have health insurance or your insurance does not cover substance use treatment services (this is uncommon).

    • A current Wisconsin photo ID
    • Proof of residency within the last 30 days (e.g., a utility bill showing your current address)
    • Pay stubs from all employment/income this year; “income” includes unemployment pay stubs and/or Social Security Disability or Social Security Supplemental Income pay stubs.

  • Drug Testing of All Intoxicated Driver Program Clients

    ALL Intoxicated Driver Program (IDP) clients will be required to provide a urine sample for an initial six-panel drug test. This screening will be done after the clinical interview during the assessment. This test is being done to help ensure an accurate assessment as well as enable better intervention upon treatment engagement. This urine test has NO bearing on your drivers license and there is no additional fee for this test.

    Any test that is diluted and/or altered will be assumed to be positive, and that information will be shared with your treatment provider. Refusal or inability to provide a sample may be considered a positive result, and that information will be shared with your treatment provider and/or other supervising entities (example: probation agent or WCS).

     

    Biomarker Testing (Fingernail Test)

    For clients attending treatment, Biomarker testing may be included in your plan of care to aid in achieving treatment goals and promote successful completion of your Driver Safety Plan. 

     

    1 or 2 OWI Offenses:

    For those who have one or two operating while impaired (OWI) offenses, one test using fingernails may be conducted before the end of the Driver Safety Plan (DSP). 

    • Clippings will be taken from all 10 fingernails, totaling no less than 150 mg, which is about 3 – 4 weeks of growth.

    • Client's are responsible for scheduling biomarker testing appointments with the Hope Council. However, tests will not take place until six months AFTER the assessment date has passed, if required.

    • The testing fee of $115 (alcohol only test) or $200 (alcohol, THC, cocaine, opioids, amphetamines, and PCP test) must be paid before the test will be completed.

     

    3+ OWI Offenses:

    For those who have three or more OWI offenses, three tests using fingernails or blood spot (PEth) may be conducted in four to six-month increments during the one-year DSP.   

    • Clippings will be taken from all 10 fingernails, totaling no less than 150 mg, which is about 3 – 4 weeks of growth.

    • Your treatment provider will review the test results to help guide your care plan, support progress toward your treatment goals, and promote the successful completion of your Driver Safety Plan.

    • The testing fee of $115 (alcohol only test) or $200 (alcohol, THC, cocaine, opioids, amphetamines, and PCP test) must be paid before the test will be completed.


    How much sample is needed for a direct biomarker test?

      Applied when appointment is not made within 14 days of conviction or failed to complete Driver Safety Plan (DSP) within year’s timeframe or comply with recommendations of the DSP.  
    Plan Extension............................................................ $100.00
      Request must be submitted by treatment provider 14 days prior to plan expiration and approved by Hope Council. Extension of up to 4 months may be granted in order to complete treatment.  
    Alcohol and Other Drug Testing.................................. $200.00
     

    For those with 1 or 2 offenses, if you are referred to treatment, you may be required to complete 1 test during your DSP.

    For those with 3+ offenses, you may be required to complete up to 3 tests throughout the duration of your DSP.

    Rescheduling and Cancellation Policy*

    • There is a $100 fee for all assessment appointments canceled with less than 48 hour notice and for no-shows.

    • There is a $25 fee for all testing appointment no-shows.

    • An appointment is considered cancelled if you are more than 15 minutes late or if you are present but will not be seen due to the determination made by Hope Council staff.

    • All fees must be paid in full before an appointment can be rescheduled.

    These fees do not include Driver Safety Plan costs or re-instatement fees with the Department of Motor Vehicles and are subject to change without notice.

     

    Assessment Appointments Policy

    It is the policy of the Hope Council to refuse assessment services to those appearing for their appointments under the influence of any mood-altering substance not used as prescribed. Should the assessor suspect that you are under the influence of ANY mood-altering substance, your appointment will be considered cancelled, and you will be required to reschedule.*

    Your full attention is needed during the assessment, and ALL our clients deserve privacy. Therefore, NO ONE else, including children or infants, is allowed to wait in the lobby or be present during your assessment. Failure to adhere to this rule will require you to reschedule.*

    Important information regarding insurance: If you have health insurance, please contact your insurance company BEFORE your assessment appointment to see what treatment providers your insurance will cover, in the event that treatment is deemed necessary through the assessment findings. If you are deemed in need of treatment, you are REQUIRED to receive that treatment from a Wisconsin Statute 75-certified agency.

    Once the appointment has taken place, the treatment provider must be contacted within 3 business days from the assessment. 

    Failure to contact the provider within 3 business days may result in noncompliance status/other sanctions.

    The Hope Council does not recommend or endorse any of the listed service providers. Inclusion on the “Preferred Treatment Providers” list means that the provider has agreed to collaborate and communicate with staff from the Hope Council, ensuring that this agency will receive timely communication regarding your progress. The provider also agrees to participate in best practice trainings and quarterly meetings and utilize best practices such as motivational interviewing and sensitivity to cultural diversity. Additionally, the provider has the ability to do both quick screen and lab-quantified levels of drug tests.

    You are responsible for interviewing and selecting a suitable treatment provider. Therefore, you must contact providers directly for relevant information such as insurance coverage and fees.

    If you chose a provider who is not on the preferred list, YOU are responsible for ensuring the Hope Council receives monthly reports.
