    Context and Process

    Final reports are required at the conclusion of the grant period and are due on or before May 1. If you are unable to submit by this date, please contact us as soon as possible to discuss an extension.

    These reports help Hospice Giving Foundation understand the outcomes and impact of the funded work, identify any challenges, and highlight key lessons learned—with special emphasis on what HG Foundation funding made possible over the past year.

    Please complete this form even if you have not yet spent the entire grant.

    If a balance remains after May 1, we will request a supplemental budget outlining how the remaining funds were used. Please contact us to discuss timing and options for this additional requirement.

    We understand that community-based work often shifts in response to evolving needs. Final reports do not need to reflect perfection—what matters most is honesty, clarity, and thoughtful reflection. Our goal is to foster shared learning and continuous improvement in the collective effort to expand access to compassionate, high-quality end-of-life care.

    We also welcome reflections, insights, and stories that illustrate the human impact of your program(s).

  • Demographical Data

    We understand that organizations collect and report demographic data in varying ways. In recognition of these differences, we have refined our approach to requesting this information. While specific data points are valuable in helping us understand how grant funds are reaching the community, we encourage you to share the data your organization currently tracks. Most of the dempgraphics questions are now optional. Services by region and other pertinent information directly related to your grant remain required fields.

    Unless otherwise specified, please report data for the fiscal year in which your grant was awarded, using the period July 1 through June 30.

    If your internal systems do not align with this timeframe, you will have the opportunity to indicate the reporting period used in a separate question.

    To support a more comprehensive understanding of your program’s reach, we added a required narrative question. Please use this space to describe your organization’s approach to collecting demographic information, including any relevant policies, practices, or contextual details that inform how you track and understand the populations you serve.

    As your original grant proposal included projected figures for the grant year, we request that you make every effort to report the actual number of individuals served. If data for the full grant year are not yet available, you may provide reasonable estimates for the remaining months.

    The income limits listed below are based on the 2024 State Income Limits Briefing Materials (California Code of Regulations, Title 25, Section 6932), published on May 9, 2024, by the California Department of Housing and Community Development. These limits are updated annually for each county using data from the U.S. Census Bureau’s American Community Survey. For more information, visit:

    We understand that many organizations do not collect income data; therefore, this question is optional. However, we are providing the updated chart below for your reference. Please note that median income levels in Monterey County fall near or  below the State’s designated low-income threshold for 2024.
