Final Reports
Context and Process
Final reports are required at the conclusion of the grant period and are due on or before May 1. If you are unable to submit by this date, please contact us as soon as possible to discuss an extension.
These reports help Hospice Giving Foundation understand the outcomes and impact of the funded work, identify any challenges, and highlight key lessons learned—with special emphasis on what HG Foundation funding made possible over the past year.
Please complete this form even if you have not yet spent the entire grant.
If a balance remains after May 1, we will request a supplemental budget outlining how the remaining funds were used. Please contact us to discuss timing and options for this additional requirement.
We understand that community-based work often shifts in response to evolving needs. Final reports do not need to reflect perfection—what matters most is honesty, clarity, and thoughtful reflection. Our goal is to foster shared learning and continuous improvement in the collective effort to expand access to compassionate, high-quality end-of-life care.
We also welcome reflections, insights, and stories that illustrate the human impact of your program(s).
