Liability Waiver and Code of Conduct Agreement
I understand that swimming and participation in activities at Mountain View Aquatic Club involve inherent risks, including the risk of personal injury, illness, property damage, or death. By participating in this program, I voluntarily assume all risks associated with the use of the pool and its facilities.
In consideration for being allowed to participate, I, on behalf of myself and my parent/guardian, release, waive, and discharge Mountain View Aquatic Club, its board members, officers, employees, volunteers, lifeguards, and agents from any and all claims, liabilities, damages, costs, or expenses arising out of or related to any injury, accident, loss, or damage that may occur while using the facility, except in cases of gross negligence or willful misconduct.
I understand that this privilege is extended to local high school athletes, band members, and auxiliary members and that I am expected to follow all pool rules, staff instructions, and standards of appropriate behavior. Any student who fails to follow pool rules, demonstrates unsafe behavior, uses inappropriate language, causes disruptions, damages property, or refuses to comply with directions from pool staff may be immediately removed from the facility.
Mountain View Aquatic Club reserves the right to revoke pool privileges at any time. Serious or repeated violations may result in suspension or permanent loss of pool access without refund, and the student may be banned from returning to the facility for the remainder of the season or longer at the discretion of club management.
I acknowledge that I have read and understand this waiver and agree to abide by all pool rules and staff instructions.
