This form is the approval to have Huntley Park District staff transport my child to/from an external program.

OPTION 1 - My child will be participating in swim lessons at Stingray Bay from 8 – 8:30a.
- Option 1A - I, the parent, will drop my child off at Stingray Bay for swim lessons and camp staff will sign my child in to camp.
- Option 1B - I, the parent, will drop my child off at camp and camp staff will sign my child in and out of swim lessons at Stingray Bay.
OPTION 2 - Please drop my child off at the external program.
- Staff will sign my child out of camp and into the external program.
- I, the parent, will pick my child up from the external program.
OPTION 3 - Please drop off and pick up my child from the external program.
- Staff will sign my child in and out of the external program.
- I, the parent, will pick my child up from camp.
