    Timeline of Participation: 

    Hotel: The deadline for changes to hotel reservations has now passed. If you wish to arrive or depart on alternate dates, please let us know. Changes can be made based on hotel availability.              

    If you have not already done so, we must have a Disclosure on file to obtain CME. If not already complete, please do so here: DISCLOSURE FORM. 

    March 9, 2026: A draft copy of any presentation is required for Accreditation/CME submission purposes.  
    Please include a disclosure slide - (list no discloures if you have none.)

  • A/V Requirements:

    The following equipment will be available in the meeting rooms:

    • Podium
    • Podium microphone and/or table microphones for panel presentations
    • Screen
    • LCD projector
    • A laptop pre-loaded with your presentation
       

    If you require additional equipment, please note it in the box below on this agreement form. If you are using a Mac computer for your presentation, please bring your computer and the necessary connector, and indicate this on the agreement form.

    We will try to honor every request if we are given advance notice.

    I assure that the materials submitted for this conference are my original works and grant permission to ICHS to print them for use at the meeting, on the association website, or in other electronic vehicles for reference purposes.

    I further agree that ICHS has the right to use, replicate, and/or display any photographs (still, motion picture, and/or video) of me, including audio or readings of my voice, in educational audiotapes, videotapes, internet, television, public displays, or advertising related to the ICHS 24th Biennial Symposium.
