Request for Educational Records
This form is for requesting educational records of a student, grades kindergarten through 12th grade, who is currently attending or has previously attended a school within the Arizona State Schools for the Deaf and the Blind (ASDB.)
As defined by the Family Educational Rights and Privacy Act (FERPA), an “education record” refers to records directly related to a student and maintained by an educational agency. These records may include, but are not limited to:
- Grades
- Transcripts
- Class lists
- Student course schedules
- Health records
- Discipline files
Important Notes:
1. If your child is enrolled in your home school district and is receiving only itinerant services from ASDB, please contact your home school district directly for any record requests. ASDB does not maintain any records for students receiving itinerant services.
2. If you are requesting records for a child (birth to 5 years old) in our Early Learning Program or other records for a student (grades K-12) attending ASDB, you must submit a public records request.
