  • Iconic Dobermans, hereby known as the Lender, agrees to rent a Holter heart monitor and accessories listed below to:

  • , hereby known as the Borrower.

  • This agreement is to be signed and kept on file for current and all future rentals and will apply to all check outs of the Iconic Doberman Holter equipment and accessories. Once a document is on file all future renters will be required to sign in and out on the club Holter sign out sheet. By signing this agreement, the borrower agrees to all its terms and conditions for any and all rentals.

    These fees cover the cost of shipping to the Borrower as well as Holter accessory replacement and maintenance. Each member who uses the Holter must have a Holter Rental Agreement on file with the Lender.

    Method of payment: All individuals and Chapter Club rental fees must be prepaid through the Iconic Doberman Paypal: brandyjbillington@yahoo.com. No payment will be accepted until the borrower has completed a Holter Rental Agreement and been accepted for rental by Iconic Dobermans.

    Rental Timelines are 3-5 days for 1 dog, then an additional 3 days per additional dog. Please ensure to select a potential start date for your rental timeline at the bottom of the agreement.

  • Reasonable use of consumables (tape) is included in the rental fee and will not be assessed against the Borrower. If necessary, any damage, cleaning or late fees will be charged to the Borrower. Should a Borrower neglect to insure the Holter package at the proper amount of $500 for return shipment or send it to the Lender’s home instead of the designated shipment location and damage or loss occurs, the Borrower is responsible for all replacement fees/damages. All Results of the Holter reading of any Iconic Dobermans are to be made available to Iconic Dobermans within 48hrs of receiving results.

    The undersigned, hereby confirms the contract particulars as noted above, subject to the terms and conditions which form a part of this agreement. This offer when accepted in writing by the Lender or Agent constitutes a legal and binding agreement.

  • Proposed Rental period is not confirmed until you are contacted with shipping information. Unexpected situations or issues can occur that may result in delays. If you have a very small window of time in which you need the holter completed, such as a breeding or emergency, please reach out to Brandy directly with your concerns and we will do our best to accomodate.

  • Canine Service Type I

    This is our basic service option for Veterinarians.  It is also the common choice for Breeders and Pet Owners who are looking for an accurate PVC screening of their dogs.  When we receive your Holter recordings, our certified Technicians will carefully scan them and generate a Hospital quality report, complete with a clear PVC summary added to the cover page.  This is a manual scan, not a computer generated scan.  The complete report will be sent to you via E-mail. Our fee for this service is $40.00 per scan for 24-hour studies.  The fee for long term studies of 2-7 days is $60 and the fee for studies of 8-14 days is $100.

     

    Canine Service Type II

    This is a more in-depth service for customers who want a Cardiologist overview of their study.  The scan is processed as in the "Service Type I" and the preliminary report will be sent to you.  This report is also sent to our Cardiologist.  The Cardiologist will then send a interpretation of the report to you.  This is an option typically used for customers who want a Cardiologist opinion regarding their dogs Holter Report.  The fee for this service is $100.  If you have already paid $40 for Service Type I, you can add the Cardiologist service for an additional $60. 
    This Cardiologist report can also be used by Doberman and Boxer owners who want an Advanced Cardiac OFA certification for their dog.  If you are interested in submitting your report to OFA for certification, then you need to submit both the preliminary report and the Cardiologist report and mail directly to OFA while clearly stating that you are submitting these items for the OFA Advanced Cardiac Database.  More information can be found by clicking here.

    IMPORTANT NOTE:  Every Holter Monitor report provided by ALBA Medical is a manually scanned, full service report.  Our canine scanning technicians are certified veterinary-specific EKG technicians with over 20 years experience in scanning Holter Monitors, and are under the supervision of a Board Certified Veterinary Cardiologist.  Our technicians spend up to 1 hour visually scanning the ECG, and accurately generating a final report.

    All studies analyzed by ALBA Medical include:

    • Supraventricular Analysis
    • Pause Analysis
    • Heart Rate Analysis

    All Holters sent in by Breeders, Vets and Cardiologists are all read the same, and have all of the analysis features.

  • ALBA - Online Holter Transmission Instructions

  • Online Holter Transmission Overview - Your ALBA Digital Holter uses a standard SD or CF flash card.  When you insert your flash card in a properly installed card reader and look at the files on that card, you will notice there is only one file on each Holter card called “flash.dat”.  The concept of online Holter transmission is to get us this file (the 24-hour recording), and also the patient demographics and other important information.  We use a file transfer company called HIGHTAIL for this file transfer.  Please note that the Holter file is too large to email, and this is why we need to use this "Drop Box".

    This online version is pretty straightforward if you have a good internet connection and a bit of computer saavy.  Every computer and Internet system is a little different, and we cannot provide extensive tech support for every situation.  If you have a problem, you are free to call us, but if we determine that our system is working correctly, you will probably be directed to send in your card.  Please click here to get a form that includes our address.


    Regarding Payment – Alba uses a company called Square to hold our credit cards on file, and to charge the cards. 

    • If ALba has your credit card and CCV in the new Square system, you will receive a paid invoice from Square, and the report will be an attachment to the invoice. 
    • If we do not yet have your card and CCV in the new system, you will receive an invoice to pay online.  After paying online, we recommend that you click the button that authorizes Square to keep your credit card on file.  After you pay the invoice, we will send you a paid invoice with the report attached.

     

    Step One: Go to our “drop box”

    Step Two: Transmit a Holter to ALBA Medical

    You will see a message that says “Drag files here to share or pick from your computer.” You can “drag” the flash.dat file from your Holter chip to the box, or you can click “computer” to open a browser.  With the browser, find your flash drive (e: or f: or similar) and then choose the file recorded on it (flash.dat).

    In the form on the left of the submisison page, please provide the following:

    1. Enter your Full Name
    2. Enter your Email (this is where your report will be emailed to)
    3. Enter Message= Your name and phone number (very important) then dog’s Name, Age, Breed, and any notes you want to tell us about the recording period. 
    4. Add which service type you are requesting (Canine Service Type 1 or Canine Service Type 2)
    5. Click “Send”.  The file will then be sent over to us in a matter of minutes.

     

    Please note: Do not erase the flash card using “My Computer” or “Windows Explorer”.  If the flash.dat file is not present, the Holter will not function.  Leave your card full.  The next time you are hooking up another patient, insert the card, and the Holter will prompt you to erase the card. 

    • Please review Report types and Instructions prior to submitting order.
