💡 Before submitting your cancellation request, please read the following:
* Your account must be in good standing (i.e., no overdue payments) for your cancellation to be processed.
* If you have any outstanding amounts, your request will not be processed until your balance is paid in full.
* All cancellations require 7 days' notice. Please note that any debits that fall within the 7-day notice will be taken in full, and you will have access to the facility for the remainder of the pay cycle.
* If you are still within a contract term, you may choose:
1. To pay out 75% of the remaining balance, to cancel immediately.
2. To continue to the end of your term, after which your membership will automatically cancel, upon completion of this form.
* You must upload valid photo ID to verify your request.
* You can check your account status in the Client Portal or the In Balance Fitness App. Android App / Apple App
