 STEP 1 Make a Tele-Consultation appointment with Dr Koh

1. Dr Koh will call you on the contact number provided in this form. He will call you from a NO CALLER ID line at your appointment time. 
2. You can only make 1 tele-consultation appointment per person. Please submit another form to book for another patient.
3. Consultation Fee is $30. This fee is waived if you are making a prescription order. 
    

STEP 2 Prescription Order & Collection 

IMPORTANT: We need 1 day to prepare your prescription. 

1. When booking your prescription, the collection time must be at least 1 day after tele-consultation.
2. Collection at: Koh Kee Chang TCM Physician, 77 Indus Road, #01-501, Singapore 160077
3. Payment methods: Cash and PayNow (PayNow to KC KOH at 91161833).
