    We’re excited to welcome you to The Base Performing Arts. This form helps us place your child in the right class so they can have the best possible first experience with us. You’ll be able to select the classes you’re interested in, and our team will review everything to make sure it’s the right fit before confirming your trial. If your child doesn’t love their first class, your trial is free. No pressure, no stress. If you have more than one child trialling, please fill in separate forms.

  • Recreational 15+ yrs is for intermediate level & above dancers 

    These classes are best suited to dancers with previous experience and an intermediate level of training or above. If you're unsure whether this is the right class for you, simply complete the form and our team will be happy to help you choose the best option.

    Please read, complete and sign the following waivers and agreements. Students cannot begin classes until these agreements are filled out. I, the parent/guardian, acknowledge that participation in dance is potentially dangerous and there is an inherent risk of injury involved. In allowing my child to participate in The Base Performing Arts activities, I hereby assume all the risks associated with the performing arts. I understand the importance of my child following the instructions and rules set by their teacher/s, and I agree to release The Base Performing Arts and its employees of any and all liability which may arise as a result of my child's participation in activities at The Base Performing Arts.I, the parent/guardian, will inform The Base Performing Arts of any underlying health issues that may affect my child during their time at The Base Performing Arts. Please consult your healthcare professional if you have any concerns before commencing classes. The instructions and advice given in a session are not to replace the advice from a health care professional. All questions or concerns regarding diet, health and weight should be directed at your healthcare professional. Your child should never exercise at a level other than one in which they feel comfortable with. Always use proper form and stability whilst performing exercises and using equipment. The use of equipment requires a certain level of responsibility and care assumed by your child. We disclaim any liabilities in connection with the exercises contained within a session.
