Team/Organization Registration Agreement

1. Purpose
This agreement outlines the terms and conditions for participation in the Strikes 4 Homelessness Bowling Fundraiser hosted by Integrated Healthcare Systems, Inc. (“IHCS”). The purpose of this fundraiser is to generate financial support for IHCS’s housing initiatives and community services for individuals experiencing homelessness.
2. Minimum Fundraising Commitment
By registering a team or organization for this event, the undersigned (“Participant”) agrees to:
Raise a minimum of $1,000.00 in donations/sponsorships in support of IHCS prior to the event date.
Submit all funds collected no later than October 4, 2025.
3. Registration & Confirmation
Registration is complete only upon receipt of this signed agreement and the official registration form.
Participation will be confirmed only after IHCS verifies the Participant’s fundraising commitment.
Teams/organizations that fail to meet the $1,000 minimum by the deadline will forfeit their spot and any registration fees paid.
4. Fundraising Guidelines
Participants may collect contributions from individual donors, corporate sponsors, or through approved fundraising activities.
All donations must be documented and submitted with a fundraising report to IHCS.
IHCS reserves the right to review all fundraising methods to ensure compliance with applicable laws and the organization’s mission.
5. Event Participation
Each registered team may consist of up to 5 bowlers per lane.
Participants are encouraged to wear team shirts or uniforms to promote team spirit and community engagement.
IHCS will provide lane assignments, scoring, and event-day logistics.
6. Liability Waiver
By signing below, the Participant releases IHCS, its officers, employees, volunteers, and partners from any liability for personal injury, property damage, or loss that may occur before, during, or after the event.
7. Agreement Acknowledgment
By signing this agreement, the Participant affirms that they have read, understood, and agree to the terms outlined above, including the minimum $1,000 fundraising requirement.
