PARENT AGREEMENT
1. CALENDAR: The EXPLORERS, after school program follows the NYC DOE calendar, with some exceptions for Jewish holiday closings. Half days are included.
2. EMAIL POLICY: I understand that I am responsible for checking the email address listed on the front of this form for information on closings, trips, special events, and other information related to the EXPLORERS program.
3. DROP-OFF & PICK-UP: Parents are responsible for arranging transportation from their child’s school. Pick-up from the JCC is at 6:00PM.
4. HOMEWORK HELP: The Explorers program offers homework help time daily, with some exceptions throughout the school year. Please note that faculty are DOE certified teachers who are available to assist your child with daily homework assignments. They are not available for private tutoring. Children are only allowed to participate in homework help with proper classroom behavior and failure to adhere to this will result in your child being removed from homework help. It is the parents' responsibility to ensure that their child is prepared with all assignments, materials, and supplies required to complete homework at the JCC.
5. DISCIPLINE POLICY: If in the opinion of the JCC staff, your child's behavior prohibits their safe and/or successful participation in the EXPLORERS program, we reserve the right to remove your child from the program. The EXPLORERS program will not tolerate any behavior which is emotionally and physically harmful to a child or staff member. Possible actions by the JCC resulting from unsatisfactory behavior include notifying the parent/guardian, a conversation with a JCC staff member, a loss of certain EXPLORERS program privileges, suspension, and removal.
6. PROPERTY: the JCC is not responsible for damaged, lost, or stolen property.
7. CHILD RELEASE: I understand that nobody else other than the parents/guardians and other person(s) listed on the front of this form are allowed to pick up my child unless I notified the JCC in advance.
8. LATE PICKUP FEE: Children must be picked up by 6:00 PM every day. If my child is picked up after 6:00 PM on a recurring basis, he or she is subject to suspension or exclusion from the Explorers program.
9. ACTIVITIES: I hereby grant permission for my child to use all play equipment and participate in all Explorers program activities. I am fully aware of all the risks inherent in various activities and will not hold the JCC, its instructors, employees, associates, agents, affiliates or any other entity or person associated with the JCC liable for any injury or loss that may occur during after school Explorers session.
10. TRIPS: I hereby grant permission for my child to leave the JCC premises, under adequate supervision by staff, to participate in special activities and trips.
11. MEDIA: I hereby grant permission for my child to be included in photographs or videos for JCC publicity. I release all rights to such photographs and videos.
