Club Liability Application

This coverage is for INDIVIDUAL teams participating in an event that need to provide evidence of liabiltiy insurance to an event organizer for a one day or multi day event. If you have multiple teams each team must apply separately.

If your team consists of members of a club that has club liability insurance with Paddlesport Risk Management you may use the additional insured request form instead of this form to request a certificate. Your team members must all be part of the club you are affiliated with.

If your team consists of paddlers from various clubs or not affiliated with a club then use this application to apply for team liability

IT IS NOT FOR EVENT ORGANIZERS. EVENT ORGANIZERS MUST USE THE EVENT APPLICATION.
