Soccer is a contact sport and, as such, injuries do happen.
Whilst JSSL Singapore will take every precaution to ensure injuries do not occur during their coaching sessions and league matches, JSSL Singapore, their coaches, staff and players will not be held responsible or liable for any permanent injuries or losses of property to your child that may occur arising from any cause whatsoever during their time at all JSSL venues, during a coaching session or a league match. JSSL Singapore, their coaches, staff and players will not be held responsible or liable for any permanent injuries or losses of property to Parents or relatives of JSSL Singapore that may occur arising from any cause whatsoever during their time at a coaching session or a match. We would advise parents to get themselves covered with your own personal insurance in relation to any sports-related injuries. Should JSSL Singapore be unable to contact the child's parents, I hereby give my permission to JSSL Singapore to seek medical assistance for my child. I will bear full financial responsibility for any costs incurred.
JSSL holds the rights to use images & videos & film of players for all JSSL marketing purposes.
JSSL holds the rights for photos, videos, or film likeness to be used for any legitimate purpose including video team analysis by the activity holders, producers, sponsors, organizers, and assigns.
The person entering this form MUST be the child's parent or legal guardian. By signing below means that you have read, and understood and agree to the RULES and REGULATIONS document and you consent to be bound by, all the terms & conditions of the agreement governing JSSL Singapore. Thank you.
