I hereby give my approval for my child’s participation in all activities organized by the Jada Cody "ITC" Softball Camp during the selected camp sessions. In consideration of my child’s acceptance to participate, I fully assume all risks and hazards incidental to the conduct of these activities. I release, discharge, and hold harmless the Jada Cody "ITC" Softball Camp, its officers, agents, employees, coaches, representatives, and volunteers from any and all liability, claims, demands, or causes of action arising out of or related to any injury, loss, or damage to person or property, including death, that may occur as a result of my child’s participation in the camp, including travel to and from the event.
In the event of injury or medical emergency, I hereby authorize the Jada Cody "ITC" Softball Camp and its representatives to seek and secure medical treatment for my child as deemed necessary. I understand and acknowledge that participation in softball and related activities carries inherent risks, including but not limited to fractures, paralysis, or death.
I agree that this release and waiver applies to all claims for damages or injuries caused by the negligence, active or passive, of the camp, its staff, or other participants, to the fullest extent permitted by law.
