    SCHOLARSHIP APPLICATION

    The JMD Angels Foundation is now accepting applications from students in the Chicagoland area.  

    We offer two scholarship options: Angel Scholarship or Sponsored Scholarship. Applicants must select only one scholarship option. You may apply for either the Angel Scholarship or the Sponsored Scholarship — not both.

    Students who have already received a JMD Angels Scholarship twice are not eligible to apply again.

    Application Deadline:

    All applications must be submitted by 5:00 p.m.(cst), Friday, April 24, 2026.

    Late applications will not be accepted.

    Award Amounts:
    Scholarships start at $500.  Maximum award amounts vary.

    Interview Requirement:
    Students selected to advance must complete a Zoom interview with the Scholarship Committee.  Business attire is required.

    TWO OPTIONS FOR SCHOLARSHIPS (JMD Angel or Sponsored):

    Applicants can apply for an Angel Scholarship or one Sponsored Scholarship.

    You cannot apply for both scholarships!

    All scholarships require an essay and transcripts.

    Upload a current unofficial high school transcript (HS seniors only) or a current unofficial college transcript. YOUR FULL NAME MUST BE ON YOUR TRANSCRIPT. 

    High school graduating seniors must upload their letter of acceptance from the college they plan to attend in Fall 2026.

    Angel Scholarship Eligibility and Requirements:

    Minimum 2.5 unweighted GPA on a 4.0 scale

    High school graduating seniors planning to attend an HBCU.  

    Rising college sophomores, juniors or seniors attending any U.S. college or university 

    Essay question: What have you done to make your school or community a better place? What did you learn from it? How has it shaped your values and future goals? 

    The essay must be one page single spaced.

    Please note that ChatGPT or Artificial Intelligence (AI) generated essays will result in an application being rejected.

    Reminder: Applicants can only apply for an Angel Scholarship or ONE Sponsored Scholarship.  You cannot apply for both!

    I certify that the information in this application is true, correct and that the essay was written by the applicant.  I understand that falsification of any information will result in disqualification of this application. I understand that this application will be assessed based on the information provided and submission of it does not guarantee that I will receive a scholarship.  I agree that in the event I am a scholarship recipient, this form also serves as a release to use my name, photographs, and biographical information in press releases, etc. announcing and promoting this scholarship program.

  •  Scholarship awardee requirements:

    1. Grant permission for JMD Angels Foundation to post their name, photo, and award amount on the Foundation’s website and social media. (Declining permission will result in forfeiting the award.)


    2.  Attend the Scholarship Awards Ceremony on Monday, June 22, 2026 at 5:30 p.m. If unable to attend, a parent or guardian must be present. Scholarship checks will NOT be mailed. The location will be announced in the near future.

    Award Notification:


    Recipients will be notified by email on June 1, 2026.
