REVersed is an educational alcohol and drug awareness program approved by the RTA and provides the opportunity to build knowledge and skills to reduce recidivism in terms of drink and drug driving behaviour. It is not a guarantee against reoffending;
1. I am required to pay the program fees of $220 or $180 for Healthcare Card holders;
2. There is a non-refundable deposit of $50 included in the fees;
3. I am required to attend each of the two sessions of the program otherwise I will not be issued with an attendance certificate;
4. I am required to be on time for the session to commence at 6.pm and if I arrive any later than 6.10 pm I understand I will not be allowed to participate in the session;
5. I am required to be alcohol and drug free when attending the program;
6. I will be breathalysed at each session;
7. I will notify the Karralika Programs Inc. office of any of the following: change of address, inability to attend a session, inability to pay the fees at the agreed rate;
8. I accept the program agreement concerning attendance and participation in program activities, including homework, respect for facilitators and respect for other participants;
9. The following information may be shared with Courts, and referring agencies: enrolment, graduation certificate, breaches of the program rules;
10. Data collected in this course may be used for statistical purposes without name identification;
11. I may be interviewed at a mutually convenient time and place sometime after the program as required;
12. There may be attendance of students at sessions for training purposes;
13. I have to provide documentation relating to sickness or work commitments if unable to attend;
14. I understand that if I miss one session I must re-enrol for a new program.
