Informed Consent & Release from Liability 

The terms of services include, but are not limited to the following:

~I understand my hair goals may require more time than available in a single appointment, and any adjustments must be requested within 7 days of my services. All adjustments are subject to additional costs, and any additional services after 7 days will be preformed at full cost.

~Allergic reactions to hair color, products, or other materials used can occur. Kendall Whitley, LLC is not liabe for any allergic reactions. Notify your stylist immediately if irritation occurs.

~I understand that internal and environmental factors such as: mineral build-up, medications, anesthsia, hormonal fluctuations, UV exposure, swimming, some hair care products, well water and other external and internal factors could lead to quicker color fadage or unexpected results.

~I accept the responsibility for explaining to my stylist my desire for specific colors, shape, and expectations for any services that will be done.

~I will not be under the influence of any drugs or alcohol during my treatment.

~I understand results are not guaranteed and all services are non-refundable.

~I have been duly informed of the risks, possible complications and consequences as listed above.

I hereby release Kendall Whitley, LLC and affiliates from any liability arising from risks that are known and/or unknown affiliated with services rendered. And therefore, I agree to allow Kendall Whitley, LLC to perform services on me.

What to know when resrving an appointment:

Here's some details, but we will follow up to make sure you know exactly how much time to reserve, and personally approve each appointment request.

• How to Arrive: It's ideal to arrive to your appointment with your hair worn how you typically style it. A smooth collar line (no hoodies, or high collars) is best. Clean(ish) hair (free of build-up like dry shampoo or root coverage spray), untied (no bun or ponytail), dry hair is best- if that's not possible just know that extra time may be required to prep hair for services. 

• ​Allow Time: You and your hair needs have my total focus during your appointment. You will recieve a custom time recommendation based on your hair goals. Please allow enough time in your day for the service you've reserved. 3-4 hours are alotted for new guests, vivids, or transformational color + care appointments. 

• Pricing: We utilize transparent, gender-free, all-service inclusive session pricing. We believe in an even energy exchange, no hidden fees for add-on treatments or gratuity.Our artist’s are educated to preform services on all hair textures: straight, wavy, curly, and coils, and hourly rates will be adjusted to offer equitable exchanges based on hair density and/or length.  

• How to Reserve: All appoinments are self-reserved. Once you've submitted this form you will be redirected to my booking link, where you can request your "New Client": consultation, haircut, or total color transformation!

•  We will have a thorough in-person consultation prior to services. Eligibility for services is dependent on in-person consultation.
