In consideration of being permitted to receive such Services, I, as the parent and/or legal guardian of the minor(s) indicated below (individually and collectively, “Minor”), on my behalf and on behalf of Minor, as well as on behalf of Minor’s other parent(s) and legal guardian(s) and/or my or Minor’s heirs, next of kin, assigns, and personal representatives (me, Minor, and such other parties, collectively, “Releasing Parties”), do hereby acknowledge, understand and agree as follows:

    1. ACKNOWLEDGEMENT AND ASSUMPTION: I, on behalf of the Releasing Parties, understand and agree that IEEE has provided information regarding the Services solely for my convenience. IEEE has no involvement with or control over actual delivery of the Services, and IEEE accepts no responsibility for the Services or for any Damages (as defined in Section 3 hereof) that may arise from my use of the Services. If I decide to use Services, I do so ENTIRELY AT MY OWN RISK and subject to the Provider’s terms and conditions of use of such Services. The use of the Services may involve risks, inherent and otherwise, known or unknown, that cannot be eliminated, including but not limited to injury, illness, death or other harm to Minor and other persons, and/or loss, theft or damage to property (collectively, the “Risks”). As between IEEE and me, I assume full and sole responsibility for all Risks with full knowledge and acceptance of such Risks.

For the avoidance of doubt, if, despite this Liability Release (the “Release”), Minor or any other Releasing Party makes a claim against any of the Released Parties arising from, or relating to, the Services and/or any act or omission of the Provider, I hereby agree to indemnify, hold harmless, and defend the Released Parties from and against any Damages incurred due to such claim.
