ACCOUNTABLE TO Practice Manager
SALARY: £24,500 – £26,000 (Dependant on Qualifications and Experience)
HOURS: 37hrs per week
TERM: Permanent
ANNUAL LEAVE: 30 Days inclusive of public holidays
LOCATION: Home working initially then post will be office based in Wishaw
General Description
The successful candidate will be responsible for the day to day management and coordination of our small ‘home care’ type service called the Mobile Support Service. We are looking for an energetic and organised individual with experience in the social care field to develop the service further. The service supports people who have a learning and/or physical disability at key points of the day with things like personal care, medication, meals and light housework.
The successful candidate will be able to provide leadership to the team of mobile support workers, working closely with the service coordinator to ensure we provide the highest quality care and support to the people we work for.
In addition to this you will work in partnership with our Technology Enabled Care Service to develop a flexible on demand care and support service in North Lanarkshire.
Although SOL base is located in Wishaw, home working is required at this time and a blended model of home/office working may be adopted in the future. Travel throughout Lanarkshire would be an expectation of the post as you will be expected to undertake home visits.
Key tasks and responsibilities
- Meet with people who need support in their own homes to assess their suitability for the service and discuss their needs
- Provide leadership to the team of support workers and the coordinator, through ongoing supervision, appraisals and team meetings.
- Work in partnership with people, their families and other important people to plan person centred care and support.
- Ensure care and support is delivered in line with peoples individual budgets
- Develop good relationships with our external partners to grow the service
- Oversea the implementation of new software designed to make the coordination of services and workforce management more efficient
- Link with SOL’s Technology Enabled Care Service to deliver a flexible on demand care and support service
- Ensure people have support at the right time
- Facilitate regular reviews to ensure our service is of the highest quality and that people wishes and preferences in relation to how they are supported are met
- Ensure new staff are thoroughly inducted in line with National Care Standards and people’s individual support plans
- Ensure staff are motivated and engaged in their work
- Support individual staff members around practice development and improvement
- Work in Partnership with the People Team to ensure that probationary reviews are carried out for new staff
- Ensure staff are competent in the use of all moving and assisting equipment i.e Hoists
- Take part in on call duties, on a rotational basis
