Use this form to advise the Trust of the decision made by your Kōhanga Reo whānau to opt in to fortnightly pays.
Before you begin

Opting into fortnightly payments is a decision to be made by your Te Kōhanga Reo whānau.
- It is not compulsory to opt in.
- Kōhanga can choose to opt in at a later date.
