  • Use this form to notify the Trust of an incident involving a Trust owned, leased or rented waka.

    Please complete and submit this form as soon as possible after the incident has occurred.

    • If the waka is drivable, please return to the tari or an alternative safe location as soon as possible and notifiy your manager.

    • Please refer to the card in the dashboard for information on who to call for a assistance, eg a tow truck.

      Ensure you remove the Fuel Card and any personal belongs before the waka is taken away.
