My signature indicates I am financially responsible for all charges on the account for my above student's instructional fees with LEAP. This is not a contract to fulfill a minimum number of sessions. We may discontinue tutoring at no charge at any time with more than 24 hours' notice before our next scheduled session.
By signing below, I indicate an understanding that the registration fee is due before the tutoring is offered and confirmed and will be paid with submission of this form with my method of payment.
If the available instructional times do not align with our availability and we indicate such prior to the start of tutoring, LEAP will refund the registration fee at my request within 30 days of the date of payment. After 30 days, we forfeit the registration fee or may use it within 1 year.
If LEAP availability aligns with the availability we indicate in our registration form, tutoring will be automatically confirmed and a confirmation email will be sent with access to our online account.
If our circumstances should change and we need to permanently discontinue future instruction, I understand I must call or email the LEAP office with more than 24 business hours to not be charged.
My signature also indicates I have read in full the entire LEAP Student Handbook and agree to these policies as well as those outlined in the handbook. I understand I may access the Handbook at any time in my online LEAP account.
