2026 Entry Form
Important information on completing and submitting your online application:
1. All applications must be typed using the JotForm online application. No handwritten forms will be accepted.
2. The applicant should carefully read through the application and entry form BEFORE filling out any part of it. The complete list of rules and regulations may be downloaded here: Outstanding Young Farm Woman Rules & Worksheet.
3. Applications must be electronically submitted to the state office by MARCH 1, 2026 at 11:59pm CST.
4. Every time you work on your application, even if you are not ready to submit, you will need to click the BLUE "Save and Continue Later" button on the very bottom of the application page. A popup box will appear and you must click "SKIP CREATE AN ACCOUNT" and you will be prompted to enter your email address.You will receive an email that contains a link that will take you back to your online application. When you return back to your online application, you will be able to make edits and revisions until submission deadline of March 1, 2026 at 11:59pm CST.
5. Once you have completed your application and are ready to submit the final version, you will need to click on the ORANGE "Submit" button on the last page of the application. You will receive a confirmation email that will include a PDF attachment of your application submission.
6. Any applications worked on and/or submitted by February 14, 2026 at 11:59pm CST will be reviewed by the state office for eligibility and application guidelines and is eligible for resubmission with corrections.
7. Any applications worked on and/or submitted after February 14, 2026 @ 11:59pm CST will be submitted on the official deadline (March 1, 2026 at 11:59pm CST) without review.
8. Disregarding the guidelines will result in disqualification of the application.
9. Submission of SIGNED entry form:
  - Print the entry form (first page) of your PDF application that you received in your confirmation email
  - Sign where indicated for applicant(s)
  - Have your parish president sign where indicated
  - Submit your signed entry form to the LFBF State Office
