Important information on completing and submitting your online form:

1. All forms must be typed using the JotForm online form. No handwritten forms will be accepted.

2. Forms must be completed and electronically submitted to the state office by MAY 1, 2025 at 11:59pm CST.

3. To save and continue later you can click on the "Save and Continue Later" button on the very bottom of this form. A popup box will appear and you must click "SKIP CREATE AN ACCOUNT" and you will be prompted to enter your email address. You will receive an email that contains a link that will take you back to your online entry. When you return back to your online form, you will be able to make edits and revisions until submission deadline of May 1, 2025 at 11:59pm CST.

4. Once you have completed the form and are ready to submit the final version, you will need to click on the "Submit" button at the very bottom of this form. You will receive a confirmation email that will include a PDF attachment of your entry submission and a link that will take you back to your online entry form should you need to make any changes. When you return back to your online entry form, you will be able to make edits and revisions until submission deadline of May 1, 2025 at 11:59pm CST.

5. To submit more than one person:
   • Once you have completed & submitted the first entry a "Thank You" page will pop up.
   • At the bottom of the Thank You page there is a blue link that reads "Click here to enter additional individuals to be recognized in the Memorial Service". 
   • You can click on that link to be redirected back to the online form and enter another individual.
   • Continue the process until you have entered all individuals that your parish would like to submit.

6. Submission of SIGNED form: 

   • Print the completed form (PDF attachement that you received in your confirmation email)
   • Have your parish president sign & date where indicated
   • Submit the signed form to the LFBF State Office
     • Scan and email signed form to megang@lfbf.org
     • Or mail to LFBF, Attn: Megan Gravois, PO Box 95004, Baton Rouge, LA 70895-9004

VIRTUAL CONVENTION: If LFBF is unable to hold the 2025 Annual Convention in-person for any reason, the events could be held virtually. In the event of virtual events, changes in timing and format of the events are likely to be required. For example, some portions of the events may take place prior to the virtual convention. Applicants will be advised as soon as possible after the decision is made.

Those memorialized must have passed away between May 1, 2024 and May 1, 2025.

• A high-quality digital photo is best and can be uploaded below.  
• A formal portrait or a vertical photo is best.
• Do NOT submit a group photo - a photo of the deceased ONLY is preferred.
