Important information on completing and submitting your online entry form:

All entry forms must be typed using the JotForm online entry form. No handwritten forms will be accepted.

The applicant should carefully read through this entry form BEFORE filling out any part of it.

Entry forms must be electronically submitted to the state office by MAY 1, 2025 at 11:59pm CST.

To save and continue later you can click on the WHITE "Save and Continue Later" button on the very bottom of each page. A popup box will appear and you must click "SKIP CREATE AN ACCOUNT" and you will be prompted to enter your email address. You will receive an email that contains a link that will take you back to your online entry. When you return back to your online form, you will be able to make edits and revisions until submission deadline of May 1, 2025 at 11:59pm CST.

Once you have completed your entry form and are ready to submit the final version, you will need to click on the "Submit" button at the very bottom of this entry form. You will receive a confirmation email that will include a PDF attachment of your entry submission and a link that will take you back to your online entry form should you need to make any changes. When you return back to your online entry form, you will be able to make edits and revisions until submission deadline of May 1, 2025 at 11:59pm CST.

Submission of SIGNED entry form:
- Print the completed entry form (PDF attachment that you received in your confirmation email)
- Have your parish president sign & date where indicated
- Submit your signed entry form to the LFBF State Office
- Scan and email signed form to megang@lfbf.org

VIRTUAL CONVENTION: If LFBF is unable to hold the 2025 Annual Convention in-person for any reason, the competitive events could be held virtually. In the event of virtual competitions, changes in timing and format of the competitions are likely to be required. For example, some portions of the competition may take place prior to the virtual convention. Applicants will be advised as soon as possible after the decision is made.
