Recommend an Applicant for a LeadingAge Washington Scholarship

This recommendation is a required component of a scholarship application.
LeadingAge Washington’s Scholarship Program supports professional development and lifelong learning for individuals working in LeadingAge Washington member communities. The program recognizes that continued education strengthens both workforce excellence and the quality of life for those served.

Scholarships are awarded to applicants who demonstrate strength in one or more of the following areas:

• Academic achievement
• Professional involvement in healthcare or aging services
• Commitment, enthusiasm, or passion for long-term care and senior living

As part of the application process, candidates are required to submit two confidential recommendations. Please complete this form based on your experience with the applicant and your perspective on their readiness and commitment to continued education.

This form must be submitted by June 19, 2026 or the individual's application will be disqualifed.
