Membership Benefits and terms
Membership Agreement.
This Skin Membership Agreement ("Agreement") is made between Leander Jayne Aesthetics ("Provider") and the undersigned individual ("Member"). By signing this Agreement, Member agrees to the following terms and conditions:
Membership Benefits.
Member will receive the following benefits during the term of this Agreement: Discounted rates on treatments as specified in the membership plan. Additional benefits as detailed in the membership plan documentation.
Membership Term
The minimum membership term is six (6) months from the date of signing this Agreement ("Initial Term"). Member agrees to maintain the membership and make all required payments for the entire Initial Term. After the Initial Term, this Agreement will automatically transition into a rolling contract on a month-to-month basis ("Rolling Contract") unless terminated by either party in accordance with the cancellation policy.
Payment Terms
The total silver membership fee is £420, payable in six (6) equal monthly instalments of £70. The total gold membership fee is £520, payable in six (6) equal monthly instalments of £87. Payments will be due on the 1st of each month. Member authorises Provider to charge the provided payment method for the monthly instalments. After the Initial Term, payments will continue to be due on a monthly basis at the same rate unless otherwise notified by Provider.
Cancellation Policy
During the Initial Term: Member may cancel the membership by providing written notice, but will be liable for the full retail price (RRP) of any treatments received at the discounted membership rate if the membership is cancelled before the end of the Initial Term. After the Initial Term: Member may cancel the Rolling Contract at any time by providing written notice at least thirty (30) days in advance. All cancellations must adhere to Provider's normal cancellation policy, which is If you cannot make your scheduled treatment date/time, please contact the clinic 48 hours prior to treatment to avoid cancellation charges. If cancelled within 48 hours of appointment 10% of appointment will be charged, No show 30% of appointment will be charged. Cooling-off Period Member has the right to cancel this Agreement within fourteen (14) days of signing without any penalty. To exercise this right, Member must provide written notice of cancellation. Default and Remedies In the event of non-payment or default, Provider may terminate the membership and seek to recover any unpaid instalments and the full RRP for treatments performed. Member will be responsible for any collection costs, including solicitor fees, incurred by Provider in the recovery of unpaid amounts.
Member Responsibilities
Member agrees to comply with all rules and policies of Provider. Member must notify Provider of any changes to contact or payment information promptly. Liability Waiver Member acknowledges that treatments involve certain risks and agrees to release Provider from any liability arising from the provision of services, except in cases of gross negligence or wilful misconduct.
Data Protection
Provider will handle Member’s personal data in accordance with the General Data Protection Regulation (GDPR). Member’s data will be used for the purpose of administering the membership and providing services. For more information, refer to our Privacy Policy https://www.leanderjayneaesthetics.co.uk/contact. Governing Law This Agreement shall be governed by and construed in accordance with the laws of England and Wales. Any disputes arising under or related to this Agreement shall be resolved through binding arbitration in Bedfordshire, England.
By signing below, Member acknowledges that they have read, understood, and agree to the terms and conditions of this Agreement.
