(1) FILL OUT THE FORM BELOW
(2) ATTACH TWO PHOTOS (OF POTENTIAL CONTESTANT) - within this online application form. Each selected contestants photos will be used to promote their performance in the talent show. By submitting photos, you are providing this consent for the contestant photos to be used.
(3) SUBMIT THIS FORM ONLINE.
RULES/GUIDELINES FOR BEING A CONTESTANT
• Each contestant must be between the ages at least age 5 through 17. Any gender and ethnicity.
• Each contestant must have a parent/guardian present at the event.
• Each contestant must perform at least one talent maximum at the event and notify us via email with two weeks, if the contestant cannot perform.
• All finalist selected by the judges will receive one award and that decision is final at the end of the event.
• Each contestant must promote this event on their social media pages prior to the date of the event.
• Each contestant must arrive at least by 12:30pm and be ready to perform during event time.
• All contestants must promote this event and encourage attendance, agreeing to use the hashtags #phillyyouthgottalent2023 #learnnpc #everydaytransitions
• Contestants should only tag LearnNpc and Everyday Transitions on Instagram social media posts relevant to their performance and participation in this event.
• Contestants will Not hold Love Exquisite Arts and Recreation Nonprofit Center or Everyday Transitions liable for any loss, damage, negligence, injury sustained on the premises at this event date.
• Contestants agree not to use any profanity, disrepectful language, offensive language or gestures during the event.
• If contestants are performing as a group, only one award will be provided per group winners, if the group is selected as an award winner. Potential contestants could be asked to do an audiition via Zoom.
• There will only be three finalist, three awards only for 1st place, 2nd place, and 3rd place, selected by the organizing judges. If the organizing judges reach a tie vote, we will hold a public vote at the event.
• Failure to adhere to the rules will result in automatic disqualification of a contestant.

All applicants will be notified via EMAIL, if they are or are not considered to be a contestant.
All applications will be considered. If selected, contestant must reply to the email invitation within 7 days after receipt of email notification.
