Fill it out regardless if we have chatted previously about it because let’s face it I deal with so many concepts, there’s a high chance I am going to forget so please humour me. Thank you.

PREFERRED TIME OF DAY TO START (large tattoos are required to do an earlier start as I don't typically work past 6pm)

HAVE YOU BEEN TATTOOED BY ME BEFORE?

1. Asshole Attitude = Banned. Respect my staff.

2. If my assistant is asking, it's because I am asking, please just cooperate, they're just doing their job.

3. No minors in the shop. Period

4. No 3rd party bookings, text me yourself.

5. All forms must be filled out properly for EVERY session. Waivers must be completed the day before appt. NO FORM = NO APPT.

6. Prices are set for size, changing your size after booking = new booking.

7.If you're sick, don't come.

8. Be on time. If you're going to be late, text me. After 15 mins with out contact, I am moving on. No shows = Banned.

9. You have 24hrs to claim an offered booking. After that someone else gets the offer.

10. Cannot make a booking without a concept, the includes token tattoos.

11. No Deposit = No booking. Deposits are non-refundable. ALL money sent before appt counts as deposit. Deposits expire after 1 year on file without a booked appt.

12. You get one reschedule with the same deposit with a valid reason. Tokens are considered deposits and same rules apply.

13. No valid deposit on file = loss of design.

14. Tokens, trades or gift cards cannot purchase merch, promos or UFG tattoos.

15. I show drawings at appt. Please do not ask me for it.

16. DAYSITS = Up to a max 5hrs. These are not guaranteed hours. Finishing/quitting ahead of time does not change the price.

17. I will not duplicate your reference photo. If this is what you want, I am not he artist for you.

18. No photo realism or tribal. Not my style at all.

19. No in person consults. This form asks all the questions and keeps record for me to view later as my memory is shit. If you want me to remember, fill out the form in full.

20. I book in advance and do not take walk ins.

21. All communication is done on the text line. No social media DMs

22. If you choose to not reads these rules and get hit with the consequences of breaking my rules, that is not my problem.

Deposits are $100 for reg rate/flash, $150 for Neo Girls and day sits, unless otherwise stated in rules.
