As a student, I understand that it is dangerous to use Uber, Lyft, and other Ridesharing services. I agree to use these services with caution and with attention to safety. I have signed this agreement freely, voluntarily, under no duress. My signature is proof of my intention to execute a complete and unconditional WAIVER AND RELEASE of all liability to the full extent of the law.

By this Waiver, I, as the parent/guardian of the above-named student, assume any risk, and take full responsibility and waive any claims of personal injury, death, damage to personal property or illegal activity associated with the use of Uber, Lyft or other Ridesharing Services by the above-named student during HAMDEN EDUX Private School Program.

I understand and confirm that by signing this WAIVER AND RELEASE I have given up considerable future legal rights. I have signed this Agreement freely, voluntarily, under no duress. My signature is proof of my intention to execute a complete and unconditional WAIVER AND RELEASE of all liability to the full extent of the law.
