Acknowledgment of Physical Risks
I understand and acknowledge that participation in physical fitness activities, training, and exercises may involve inherent risks, including but not limited to: falls, sprains, muscle strains, cardiovascular stress, or other injuries. I voluntarily assume full responsibility for any risks, injuries, or damages that may occur as a result of my participation in this program.

Voluntary Participation
I confirm that my participation in Lifeline’s training sessions and related activities is entirely voluntary. I agree to listen to the instructors, follow all safety guidance, and modify any exercises as needed for my health or ability.

Release of Liability
I hereby release and discharge Lifeline, its coaches, staff, and affiliates from any and all liability, claims, demands, or causes of action that may arise from my participation in this program, whether caused by negligence or otherwise. This release includes personal injury, property damage, or loss incurred during or after participation.

Emergency Medical Authorization
In the event of a medical emergency, I authorize Lifeline staff to seek emergency treatment on my behalf and understand that I am responsible for any resulting medical costs.

Media Consent
I give permission for photos or videos taken during training to be used for promotional purposes by Lifeline (social media, website, print, etc.)

1 CLASS POWER PASSClass Package Terms & Conditions By purchasing the 1 CLASS POWER PASS you agree to the following terms: 1. Package Requirements, Details & Pricing Required Action: At the time of purchase, customers must indicate the specific date they intend to attend class. Rate: The package price is $15.00, which covers admission to one (1) fitness class: one (1) Tuesday class OR one (1) Wednesday class OR one (1) Thursday class.. Expiration: Package is valid for one (14) DAYS from the date of purchase. Any sessions not used within this 14-DAY window will be forfeited. 2. Rescheduling Policy We understand that plans change. You may reschedule your class subject to the following: Window: The new date must still fall within the original 14-day validity period from your purchase date. Availability: Rescheduling is subject to class capacity. We recommend rescheduling as early as possible to secure your spot. 2. Refund Policy Non-Refundable: All purchases are final and non-refundable. Class Discontinuation: In the event that the class program is permanently discontinued by the instructor, a prorated refund will be issued for any remaining unused sessions in your active package. 3. Schedule Changes Standard Schedule: Regular classes are held every Tuesday, Wednesday, and Thursday; 9AM to 10AM. Temporary Adjustments: We reserve the right to temporarily shift class days due to holidays, scheduling conflicts, or emergencies (e.g., moving a Wednesday session to a Friday for one week). Resumption: Following any temporary shift, the schedule will resume its regular Tuesday, Wednesday, and Thursday rotation the following week. 4. Stay Updated Real-Time Schedule: It is the participant's responsibility to check the Official Fitness Page for the most current schedule. Daily Updates: The schedule is updated daily to reflect any venue changes or time adjustments. Please check the page prior to attending your session. 5. Package Stacking If you purchase more than one package at a time, they will run back-to-back. The expiration timer for your next package only starts once you have finished the classes from your current one.
