PARENT/GUARDIAN CONSENT, MEDICAL AUTHORIZATION & LIABILITY WAIVER
1. Consent to Participate
I am the parent or legal guardian of the athlete identified in this registration form. I voluntarily give permission for my child to participate in the Lifeline Warriors Flag Football Program, operated by Lifeline Youth Sports.
I understand that participation may include practices, conditioning, athletic training, league games, tournaments, team activities, and other program-related events.
2. Acknowledgment of Risks
I understand that flag football and other athletic activities involve inherent risks, including but not limited to:
Falls, collisions, and physical contact with other participants
Sprains, strains, bruises, and fractures
Concussions and other head injuries
Heat exhaustion, dehydration, and weather-related conditions
Injuries resulting from equipment, playing surfaces, or athletic activities
Other serious injuries that may occur during participation
I understand that although Lifeline Youth Sports strives to provide a safe environment, injuries and accidents may occur despite reasonable safety precautions.
I voluntarily consent to my child's participation with knowledge of these risks.
3. Medical Authorization
In the event of an injury, illness, or medical emergency involving my child, I authorize Lifeline Youth Sports coaches, staff, or designated representatives to contact emergency medical services and seek appropriate medical assistance when necessary.
If I cannot be reached promptly, I authorize emergency medical personnel to provide treatment permitted by applicable law.
I understand that Lifeline Youth Sports does not guarantee the availability of medical personnel at every practice, game, or event.
I accept responsibility for medical expenses associated with my child's treatment to the extent permitted by law.
4. Health and Medical Information
I certify that I have disclosed all known medical conditions, allergies, physical limitations, medications, and accommodations relevant to my child's safe participation.
I agree to notify Lifeline Youth Sports if my child's medical condition or participation restrictions change during the season.
I understand that I am responsible for determining, in consultation with an appropriate healthcare professional when necessary, whether my child is medically able to participate.
5. Assumption of Risk and Liability Release
I acknowledge and voluntarily accept the inherent risks associated with my child's participation in Lifeline Warriors Flag Football.
To the fullest extent permitted by applicable law, I agree to release Lifeline Youth Sports and its directors, officers, coaches, employees, volunteers, and authorized representatives from claims arising from the ordinary risks of participation, including claims involving ordinary negligence, to the extent such claims may lawfully be released.
This provision does not release any person or organization from liability that cannot legally be waived, including liability for gross negligence, reckless misconduct, or intentional wrongdoing.
I understand that the enforceability of this provision is subject to applicable law.
6. Parent/Guardian Responsibilities
I agree to:
Ensure my child follows program rules and safety instructions.
Provide accurate emergency contact information.
Notify coaches of injuries, illnesses, or medical restrictions.
Ensure my child wears required equipment.
Support respectful behavior toward coaches, officials, teammates, and other participants.
Arrange appropriate transportation and supervision before and after scheduled activities.
