How to complete this form
The Emily Hensley Award 2020 Nomination Form is not intended to be a difficult nor a time consuming task. It shouldn't take more than 60 minutes and as a guide, we recommend that each answer should be approximately 300 words.

onsider the Eligibility and Selection Criterion,
Think about how the MGGS values are able to be addressed,
Research and fact check the Nominee and their achievements to date,
Interview the Nominee so that you can write authentically about their accomplishments and personal motivation for what they do.
Basically, the Emily Hensley Award Selection Committee want to get to know them. What you provide will assist in helping them do that. Share what you have learnt about their achievements, the reasons why you believe they are worthy of the Award, and the kind of person they are. Use your submission to bring them to life.
We want your words, so keep it personal and reflect upon experiences and achievements you are familiar with. What do people say when they think about this person? What elements of their personality stand out, and how have they made a positive impact and inspired others?
If you are not sure where to insert certain information, put it where you think fits best. The important thing is that it is included. The Supporters you include are also important as they need to be able to validate your submission and have first hand insights into the achievements and impact that the Nominee has made on others.
Finally, if you are self-nominating, this is not a time for modesty! We want to know all about your achievements - you may have received other awards, or your colleagues or mentors have encouraged you; things that made you proud. We look forward to hearing all about you.
